Over the last several years there have been challenges in the various state legislatures against the physical therapy profession via legislation promoted by chiropractors attempting to prohibit qualified physical therapists from performing spinal manipulation – a technique that has been part of the PT scope of practice since its inception.

Spinal manipulation is not designated as being under the exclusive domain of any one specific profession or group of practitioners. Physical therapists, chiropractors, medical doctors, and osteopathic physicians are all educated and trained to employ manipulation within the scope of their respective licenses and in a manner that protects the public’s health, safety and welfare. It is inappropriate for one profession to attempt to “own” a specific technique or dictate clinical practice through such legislation.

This Web page is designed to provide information and facts about physical therapists and spinal manipulation. In addition the information will contained this Web page will explain the difference between spinal manipulation and a chiropractic adjustment as well as the difference between the PT and chiropractic professions.

Please take a moment to review the information and resource materials below and learn why efforts to prohibit PTs from performing spinal manipulation would eliminate patient choice, remove competition in the health care system, and be a disservice to the patients we serve.

ik zie de relatie niet zozeer tussen het door jouw geplaatste stuk en de openingspost (Poll).
De poll gaat over de duur en vorm van opleiding tot (gespecialiseerd) fysiotherapeut.
Het stuk tekst (van APTA) gaat over het al dan niet terecht onderscheiden van voorbehouden handelingen.

Afgezien daarvan, zou ik pleiten voor een opwaardering van het huidige curriculum van de opleiding fysiotherapie. Het niveau van de huidige generatie afgestudeerden stelt ze nauwelijks in staat om op een verantwoorde wijze zelfstandig patiënten te behandelen (laat staan om keuzes te maken over specifieke behandelvormen als HVT manipulaties, of deze zelf uit te voeren).

Een opwaardering van de opleiding (en de inhoud van het daarbij horende curriculum) tot universitair niveau, zou een enorme belangrijke stap kunnen zijn in het profileren en positioneren van het vak fysiotherapie.

Binnen zo’n nieuw te vormen universitaire opleiding zou je kunnen denken aan verschillende afstudeerrichtingen (zoals sport, bekkenproblematiek, wervelkolomgerelateerde klachten, chronische aandoeningen, etc.).

Juist deze specialisatie (binnen de basisopleiding) kan leiden tot een hoger niveau van de afgestudeerde en daarmee hopelijk hoogwaardige zorgverlening.
M.i. is het niet haalbaar, noch wenselijk om iedere fysiotherapeut het volledige scala aan fysiotherapeutische behandelingen aan te leren(dus ook niet iedere FT moet gaan manipuleren of prikken of inwendige handelingen verrichten).

Haast vanzelfsprekend behoort bij zo’n opleiding ook een langere studieduur, om de gewenste einddoelstellingen te behalen. Denk aan eerst 3 of 4 jaar basis, waarna 2 jaar specialisatie.

Er zit echter een link tussen de Poll en de APTA-stuk, hier gaat het ook, indirect, over de opwaardering van de opleiding Fysiotherapie; Er bestaat nu een medewerking tussen de American Academy of Orthopaedic Manual Physical Therapists (AAOMPT) en de American Physical Therapy Association (APTA) over dit specifieke kwestie, de high-velocity thrust manipulations. Het is maar een voorbeeld van wat kan ons ten duur ook betreffen, zoals het is al gebeurd voor andere zaken uit de amerikaanse Fysiotherapiewereld. Voor de duidelijkheid: ik ben niet een voorstander van onveilige praktijken door onopgeleid/onervaren therapeuten dus hier gaat er niet om.

Effen terug naar je post: Een universitaire vorm zou volgens mij de beste keuze zijn, maar ook een 5-jarige HBO – met verplichte niveau master2 als gevolg.
Beide opties verhogen de huidige niveau en bieden vroegere mogelijkheden tot specialisaties.
